Patient Forms, Map and Directions to Middleton Office
Thank you for taking the time to learn a little bit about our office and where to start as a new patient.
At your initial visit we require you to have a comprehensive new patient exam, at this appointment you will spend an hour with one of our doctors. This way you have a chance to get to know them and they have time to go through all of your initial forms with you. The assistant will start by collecting basic information from you on past dental work and will complete a series of x-rays.
After reviewing your medical and dental histories, the doctor will begin with a head and neck exam, where they will assess the muscles and lymph nodes as well as the joint in your jaw. They will also perform an oral cancer screening. Lastly, they will update your patient chart to include existing dental work and diagnose any new treatment needs you may have.
Dr. Lord and Dr. Rypel are excellent dentists that are here to help you with all of your dental needs. They welcome questions and concerns and are always happy address those at any time.
